What an audit trail is supposed to answer

When someone reviews an AI action later, they are usually asking four things: what was requested, what context the system used, what action it took, and who approved or changed the path if the action was consequential.

If the trail cannot answer those questions quickly, it is too weak no matter how many events were technically logged.

Too much logging is its own failure mode

A flood of raw events is not the same thing as a useful audit trail. Good auditability is selective. It captures the evidence chain around the meaningful step and makes the approval boundary visible.

Put the record where operators already work

Teams review audit trails faster when the record is linked back to the request, the ticket, the payment run, the contract record, or the system artifact they already use. If the evidence lives only in a separate debug console, it becomes harder to trust and harder to use.
